New claims can be reported by phone, fax, mail or email, 24 hours a day, seven days a week, 365 days a year. Please include, if available, the name, address and phone number for all relevant parties, the date and time of loss, the location of the loss, a description of the loss, and any damage information.
If reporting by phone, please be sure to indicate if the claim involves a fatality, serious bodily injury, pollution spill, or requires immediate response.
